Western Washington’s four CHW programs are focused on hypertension, diabetes and colorectal cancer prevention, prioritizing BBIPOC neighbors and the Latinx community.

Eastern Washington and Montana


Eastern Washington and Montana’s four CHW programs are focused on closing care gaps, providing resources and supporting the mental health needs of neighbors, prioritizing Latinx and indigenous populations.

Oregon


Oregon’s three CHW programs are focused on children with specialized health needs and early childhood supports, prioritizing Latinx and BBIPOC neighbors.

Northern California


Northern California’s two CHW programs are focused on supporting communities experiencing houselessness, including equitable access to social supports and resources, prioritizing the BBIPOC communities and households with lower incomes.

Southern California


Southern California’s five CHW programs are focused on equitable access to care, chronic disease self-management, mental health supports, prioritizing BBIPOC communities and households with lower incomes. One workforce development program is training new CHWS and offering internship placement in the community.

Texas/New Mexico


Texas and New Mexico’s program is focused on hypertension and diabetes, prioritizing the Black and Latinx populations

Alaska


Alaska’s CHW program is focused on hypertension, prioritizing BBIPOC neighbors.
